Nurse Practitioner NP, Part-Time, CCM - Tallahassee, FL New

Hueman is hiring a Nurse Practitioner to complete care management visits in the home of members throughout Leon County, FL. These visits are to provide longitudinal, intensive, patient-centric, in-home care to the most complex and chronically ill patients. Our Nurse Practitioners are part of a multidisciplinary team and serve on the frontlines delivering preventive and urgent medical care in the homes of complex, frail, and vulnerable patients.

Nurse Practitioner Job Education and Experience Requirements:

• Active, unencumbered RN and NP license in the state of practice
• National board certification in adult, family, or geriatric specialties
• Has an active DEA license
• Current BLS for medical provider
• Must have a valid state drivers license and automobile insurance
• Strong computer skills with knowledge working in an Electronic Medical Records system
• Experience in the care of adult, chronically ill patients preferred
• Must be willing to travel
